ALL ATHLETIC PLANS |
| A student/athlete is covered as a
representative of a member school while practicing for, competing in, or
traveling to and from any interschool athletic activity under the
regulations and jurisdiction of the School and the state high school
association/league or college state or national organization, which are
under the direct supervision of a full-time employee. Coverage includes managers, trainers and cheerleaders. |

INDIVIDUAL VOLUNTARY PLANS |
| We offer a wide variety of individual
health and accident plans for students from age 4 through college. The accidental injury plans for pre-school through 12th year may be purchased for school-time coverage or for around-the-clock coverage. We offer a wide selection of plans and coverages. The accidental injury plans for college students can be purchased by the semester or term and coverage is only for classtime activities. The sickness and illness insurance plans for students age 4 through high school also covers non-athletic injuries. Premiums are paid annually or every two months. The sickness and illness insurance plans for college students also covers non-athletic injuries. Premiums are paid for the school term (Fall, Spring, Summer). Dental coverage with all these plans is for accidental injury only. |

HOME SCHOOL ASSOCIATIONS |
| This plan is available to Home School Associations
that want to provide benefits on all participants while participating in
association activities, activities like board meetings, science fair
field trips, sports days, music days and other activities are covered
for one year. A Liability Insurance Plan is available to associations that purchase the accidental injury coverage. The plan provides coverage on the Board of Directors, the Association and additional insureds. |

CATASTROPHIC ACCIDENT INSURANCE $5,000,000 Lifetime Medical Expense Benefit |

| ELIGIBILITY: | The School or School District selects one
or more of the following Classes. |
| CLASS I | Covers all student athletes, student
managers, student trainers, student coaches and cheerleaders while
taking part in the sponsored and supervised interscholastic athletic
program of the enrolled School or School District. |
| CLASS II | Covers all students of the enrolled
institution while attending such School or School District during normal
class time, and/or while taking part in its sponsored and supervised
activities, other than its interscholastic athletic programs. |
| CLASS Ill | Covers all students while taking part in
the sponsored and supervised non-athletic clubs or societies of the
enrolled School or School District. NOTE: Class Ill is included if Class
II is selected. |
| COVERAGE: | The Plan provides payments for injuries
that occur while taking part in an activity described above. |
| Coverage is also extended to students who are traveling, other than to and from school, as a member of a group under the Enrolled School or School District's supervision directly to or from such activity or while on a field trip sponsored by the Enrolled School or School District. Aircraft coverage is limited to riding as a passenger and not as a pilot or member of the crew in any Scheduled Airline Aircraft or U.S. Military Airlift Command Aircraft. | |
